External Radiation Therapy as Salvage in Patients with Post Radical Prostatectomy Prostate Cancer in Biochemical Relapse, 10-year Experience

Abstract

OBJECTIVE: To present the results of our experience during the period 2010-2020 in patients with radical prostatectomy in biochemical relapse using external radiotherapy as rescue treatment, in the GURVE Oncological Radiotherapy services. METHOD: 119 patients were selected during who met the inclusion criteria, and were treated with external radiotherapy as rescue. RESULTS: The median age was 66 years. In relation to the risk classification, the Intermediate Unfavorable represented 38.7% of the patients, Intermediate Favorable 28.6%, Low risk 15.1%, High 14.3%, Very High and Not Accurate 1.7 % respectively. The most used irradiation technique was VMAT with 94.1%, followed by IMRT with 5.0%, and with a lower percentage with RTC3D 0.8%. 83.2% of patients did not present acute gastrointestinal complications, 12.6% presented Grade 1 complications and 4.2% Grade 2. Genitourinary complications were present in 68.1% (Did not occur), 28.6% Grade 1 and 3.4%, Grade 2. Overall Survival  at 5 years is 98.5% +/- 1.5%, and at 10 years it was 92.4%, +/- 7.6%. Local Relapse Free Survival (LRFS) at 5 years was 98.6%, +/- 1.4%; and at 10 years 86.2%, +/- 6.1%; Cause-specific survival (CSS) at 5 years was 100% and at 10 years 97.3%, +/- 2.7%. Distant Relapse Free Survival (RDFS) at 5 years was 98.5%, +/- 1.5%, at 10 years 74.4%, +/- 24.6%. CONCLUSION: our results demonstrate that the administration of Radiotherapy as rescue treatment in post-radical prostatectomy patients in biochemical relapse is a safe and effective treatment in Local Control (LC) of the disease, OS and Cause-Specific Survival.
