Introduction: A pediatric hyperinflammatory disorder has recently been described, the Pediatric Inflammatory Multisystemic Syndrome associated with COVID 19 (PIMS-C). These patients generally present with fever and mucocutaneous manifestations similar to Kawasaki disease. Frequently they have cardiac involvement ranging from atrioventricular block to severe ventricular dysfunction. Case report: 7-year-old male patient who presented with abdominal pain and fever. He had clinical findings and inflammatory markers suggestive of PIMS-C and IgG serum antibodies for SARS CoV2. An electrocardiogram (ECG) showed a first-degree atrioventricular block that resolved with specific treatment for PIMS. Conclusion: Knowledge of ECG manifestations of myocardial compromise in PIMS-C can contribute to early diagnosis and treatment, avoiding life-threatening complications.
