Uterine fibroids are the most common benign tumors, however their retroperitoneal location is unusual, and the most frequent retroperitoneal tumors are malignant, so a differential diagnosis must be established. We present the case of a 43-year-old female patient with a finding of retroperitoneal parasitic leiomyoma, with no history of prior myomectomy or laparoscopic intervention. The parasitic leiomyoma lacks anatomical connection with the uterus, the pathogenesis is not clear, several explanatory models have been proposed, one being the separation of a pedunculated leiomyoma from the uterus by obtaining a blood supply from another part of the abdomen. Retroperitoneal parasitic leiomyoma is a rare entity, with a non-specific presentation, which can occur if there is a previous history of myomectomy. Differential diagnoses must be made in order not to give false expectations to the patient with a benign diagnosis due to the high frequency of malignancy in retroperitoneal tumors, it is necessary to request tumor markers, the definitive diagnosis is made with histological and immunohistochemical studies, the treatment is surgical aimed at tumor resection.
